The forms are asking for this information to make sure that all creditors are treated fairly. The court wants to see which creditors you paid more than $600 to in the 90 days before filing to make sure that they didn't receive more than they would have been entitled to receive as part of your Chapter 7 bankruptcy.
You can still file Chapter 7 even if you paid more than $600 to one or multiple creditors. The payments listed here are often for secured debts (e.g. car loans, mortgage) or non-dischargeable unsecured debts (e.g. student loans and recent tax debts).
