You should list your name the way it appears on your photo ID and Social Security card. You should also list any other names you’ve used in the last eight years, including:

• Your maiden name if you were married or divorced and changed your name as a result
• Nicknames you use frequently enough that your creditors use them to identify you
• Any other names you’ve used in an official capacity in the last eight years
